Breast reduction
Often covered when documented symptoms and the volume of tissue removed meet the carrier’s criteria.
Gender-affirming breast surgery
Frequently covered; requirements vary by plan and are confirmed before scheduling.
Implant removal for medical reasons
Coverage is possible for rupture, capsular contracture, or other medical indications.
Functional rhinoplasty / septoplasty
The breathing-related portion may be covered when there is a documented airway obstruction.
Functional eyelid surgery
Upper blepharoplasty can be covered when excess skin obstructs the visual field, documented by a visual-field test.
