- Breast reconstruction after mastectomy
- Covered under federal law (the Women’s Health and Cancer Rights Act), including surgery on the opposite breast for symmetry.
- Breast reduction
- Often covered when documented symptoms and the volume of tissue removed meet the carrier’s criteria.
- Gender-affirming breast surgery
- Frequently covered; requirements vary by plan and are confirmed before scheduling.
- Implant removal for medical reasons
- Coverage is possible for rupture, capsular contracture, or other medical indications.
- Functional rhinoplasty / septoplasty
- The breathing-related portion may be covered when there is a documented airway obstruction.
- Functional eyelid surgery
- Upper blepharoplasty can be covered when excess skin obstructs the visual field, documented by a visual-field test.
