Hairline lowering, also called forehead reduction or hairline advancement, shortens the visible forehead by advancing the hairline forward through a trichophytic incision along the front of the hair. The procedure is appropriate for patients with a naturally high forehead or recessed hairline who want a more balanced facial proportion. It can be performed alone or in combination with a brow lift. Candidates need adequate scalp laxity to allow the hairline to advance, which is assessed at consultation.

How it's done.

Through a trichophytic incision along the front of the hair, the scalp is advanced forward and the high forehead skin is removed. The incision is closed in a way that allows hair to grow through the scar for camouflage.

How much can the hairline move forward?

Typically one to three centimeters in a single operation, depending on scalp laxity. Patients wanting more advancement need a tissue expander placed first, which is a staged procedure.

Will I have a visible scar?

There is a scar at the new hairline, but the trichophytic technique allows hair to grow through it. In most patients the scar is hidden within the hair once healed.

Is this just for women?

No. Men with a high forehead are also candidates, though the planning is different because male hairlines are typically more recessed by design.

Will hair grow through the scar?

Yes — that is the point of the trichophytic technique. The incision is angled so hair follicles in the scalp grow through the scar, which camouflages it once healed.

Can this be combined with a brow lift?

Yes. Many patients have hairline lowering and brow lift in the same operation. The two share the surgical approach and produce a coordinated correction of the upper face.

What if my scalp isn't loose enough?

If a single-stage advance won't cover the desired distance, a tissue expander is placed first — left in place several months to stretch the scalp — then the lowering is performed. Staged but reliable.
