A woman in her 40s was bothered by tired-appearing eyes and asymmetric brows, and wanted to look refreshed without looking done. Dr. Azadgoli performed a temporal brow lift, an upper eyelid lift (upper blepharoplasty), microfat transfer to the face, and CO2 laser resurfacing. The main change is in the droopy left eyelid and brow. Photos show the upper half of the face at the patient's request.

Why this approach

The decisions that shaped this surgical plan.

  • Combination case planned end-to-end. Each component was sequenced to support — not compete with — the others, and recovery is timed off the longest piece.

Pre-op preparation

What to do before surgery. Specific to this case.

  • Stop NSAIDs, aspirin, and high-dose fish oil two weeks before surgery; the surgical team provides a full medication list at the pre-op visit.
  • Full nicotine cessation 6 weeks pre- and post-op. This includes vapes and nicotine replacement products; the constriction effect is the same.
  • Sleep elevated for the first week. Stock the kitchen with low-sodium soft foods, since chewing comfort and salt-driven swelling both matter early.
  • Day-of transport and an overnight companion (24 hours minimum) are required for discharge.
  • 2 additional procedures planned — plan for at least 2 weeks of in-home help rather than the standard 1 week.

Recovery timeline

Milestones specific to this case. Individual recovery varies.

  1. Day 1–7

Sleep elevated, gentle cold therapy, and quiet voice for the first 3 days. Bruising peaks midweek; swelling is at its most visible.

  2. Week 2

Stitches removed, residual bruising fading, social comfort returns. No lifting, no straining, no exercise yet.

  3. Week 4

The "I look like myself again" milestone for most. Camera comfort returns. Light cardio and yoga added back.

  4. Week 6

Recovery sequenced around the longest-component procedure. Full clearance may extend to week 8.

  5. Month 3

Final swelling resolves on the deeper tissue planes. Most aesthetic refinements emerge between months 3 and 6.

  6. Month 6

Reconstruction or combo result reaches its mature appearance. Any planned refinement step is timed for this window.

Specific to this case
  • Aftercare

Recovery sequenced around the longest-recovery component; help at home recommended for the first 2 weeks.

    Combination procedures stack recovery demand.

  • Skincare

Medical-grade skincare reintroduced at week 3 — gentle retinoid alternative first, full actives by month 2.

Active skincare resumes on a healing-friendly ramp after facial surgery.
